People with Disabilities

Our services help people live in their communities as independently as possible. We work to make sure people, and their families know what resources are available. For person's 65 or older, please see the Senior Services page.

Three ways to qualify:

Have an intellectual developmental disability and related conditions
Have a physical disability
Have a mental health disability

Access to our programs and services can be dependent on your Medical Assistance eligibility. To find out if you or someone in your care is eligible, visit the MNsure website.

There are many programs to help a person thrive in the community. Because each person is unique, we start with a MnCHOICES Assessment to determine if our programs are right for you.

Program options

A program to assist people with physical and behavioral supports in the home and community. A person must have a MnCHOICES Assessment to see if they qualify for the services. Medical Assistance is also required for the program.

Waiver services provide people and their families with access to services and supports to thrive in the community.

To qualify the person must meet three eligibility requirements:

Open or eligible for Medical Assistance
Have a Certification of Disability (federal or state)
Meet program criteria determined by a MnCHOICES Assessment. The MnCHOICES Assessment is completed by the county the person resides in.
