Senior Services

​​Our services help seniors live in their communities as independently as possible. We work to make sure seniors and their families know what resources are available.

Access to our programs are determined by Medical Assistance or other financial eligibility. To find out if you're  eligible for Medical Assistance, visit the Minnesota Department of Human Services website.  Contact us to determine your eligibility for programs the county offers.

There are many programs to help a person thrive in the community. Because each person is unique, we start with a MnCHOICES Assessment to determine if our programs are right for you. If they are, a case manager will identify your unique needs, coordinate services and provide ongoing support.​

Programs available

Alternative Care is for seniors with low income and assets who are not eligible for Medical Assistance. It provides many of the same services as the Elderly Waiver program.

Services can include help with eating, bathing, dressing and personal hygiene.

The Elderly Waiver (EW) program provides support services for seniors who qualify for Medical Assistance and need services to remain independent at home or an assisted living.

The Essential Community Supports Program is for seniors with low income and assets who are not eligible for Medical Assistance and provides a limited set of services.

Services available

Provide a range of activities including outings, social events, meals or snacks, assistance with personal care and health monitoring. Typically, the services are provided Monday through Friday, half days or whole day options are common.

Residential services such as customized living services, foster care or other type of residential care.

Services include heavy household chores such as washing floors or windows, moving heavy items and seasonal yard work.

Devices that can be worn by persons to access help in an emergency.

Products provided through medical supply companies. Options range from canes, walkers, grab bars, incontinence supplies, wheelchairs and lift chairs.

Provide help to persons who are unable to prepare their own meals.

Services include meal preparation, bed making, laundry, dusting, vacuuming, shopping and errands.

Provides services ordered by your physician. To find a home care provider, contact the Minnesota Home Care Association.

Provides a break for caregivers.
