Refer a family to WIC
If you would like to refer a patient to the WIC Program, complete this online form, and a WIC staff person will contact the family.
Families can call 952-891-7525 to schedule a WIC visit.
Formula requests
If you have questions regarding WIC-approved formulas, call 952-891-7525 to talk to a registered dietitian.
WIC may provide medical formula when requested in writing by a health care provider. Please complete all sections of the state's Request Medical Formula Form and fax to Dakota County WIC office at 952-891-7568.
