Who needs to apply?

​Uninsured residents can apply for the Behavioral Health Fund. Residents with private medical insurance can apply for the fund to be their secondary payer.

Residents using Medical Assistance or have an active PMAP do not need to apply.​​

Applying for Behavioral Health Fund via direct access

To apply for an assessment, applications can be done by:

  1. Download and complete a​ Behavioral Hea​lth F​und Request Form.
  2. Email it to dhs.bhf@state.mn.us.

After you apply

Once the Department of Human Services (DHS) receives the application, it will be reviewed and processed within five business days. If approved, a 60-day eligibility span will start to access substance use services. You will need to reapply every 60 days unless you Medical Assistance or a PMAP become active, or the services are no longer needed. ​
