Renewing Health Care Assistance
Most people who receive Medical Assistance and MinnesotaCare will have an annual renewal. To find out your renewal month, go to the Minnesota Department of Human Services Renewal Lookup Tool.
Request renewal forms online
Residents can request the following Department of Human Services forms with the online request form.
Complete an online form
You can complete forms online from the DHS website. You can either email completed forms to eeadocs@co.dakota.mn.us or print forms and send to:

Update your contact information
It's important that we can reach you when it's time to renew your health insurance. Make sure we have your current contact information. Contact your health plan to update your information.
Figuring out your renewal month
Enrollees with a Medical Assistance health plan can contact their health plan to find out when their renewal is due.
