The Minnesota Department of Health (MDH) licenses and inspects food and beverage, pool and spa, and lodging establishments in Dakota County.

Report an illness
Use this form to report a foodborne or waterborne illness (commonly called food poisoning) for yourself or someone else in your household.

You can also report a concern by doing one or more of the following:
- Call 1-877-FOOD-ILL (1-877-366-3455).
- Email your name and phone number to health.foodill@state.mn.us.
- File a restaurant or food service complaint using the MDH online complaint form.
- File a retail food business (i.e., bakeries, grocery and convenience stores) complaint using the Minnesota Department of Agriculture online complaint form.
- Submit any other questions and concerns related to lodging establishments using the MDH licensing contact us form.
